Syllabus Check Off List

1. Campus, session and year, course name.
2. Instructor: name and title, office room number, phone number or voice mail box, e-mail address, and office hours.
3. Course: name and catalog description, credit hours, and prerequisites.
4. Educational Materials: textbook title, edition, author, supplemental materials (if needed); resources--name and locations (SRC, BitLab, etc.)
5. Evaluation: examinations, quizzes, projects, assignments, papers, book reviews, field trips, types of final examination, and calculation of final grade
6. Classroom Policies: attendance, make-up examination procedure (if any), academic honesty, and student code of conduct.
7. Course Goals: course goals should be directly related to the Common learning syllabus for the course.
8. Performance Objectives: The detailed competencies (educational outcomes and related outcomes measures for your course).
9. Withdrawal date.
10. Disability Statement: Notifies students with a physical or learning disability that Cedar Valley has special services to assist their learning.  Students should also notify the instructor during the first class of the semester.
11. Disclaimer: Changes may be made at the discretion of the instructor.
12. Schedule of Classes and/or Labs: Attach the daily or weekly schedule of class, topics, reading assignments, and test dates.
